The NTSB's report into a shocking death at the Powell Street BART station in 2021 has been released.

Just before the train departed, though, Adams decided to hop off the train. When she exited the car, her dog, leashed to her backpack, stayed on board. The doors closed, and Adams was dragged down the platform until she struck a gate. She died of multiple blunt force traumas.reveals more details about the alarming incident.
The driver reported the yellow safety strip appeared clear when he left the station and no external “trouble lights” were on to warn that a door was open on the train. The driver also passed a toxicology test for drugs and alcohol after the incident, was not using his cell phone at the time of the accident, and was reportedly well-rested and had no history of prior incidents.
Adams’ autopsy showed the presence of methamphetamine, cocaine and fentanyl in her system, the NTSB said."Based on postmortem toxicology results, it is likely that the passenger was experiencing at least some impairing effects of substance use at the time of the accident,” the report said. “However, NTSB investigators found no clear evidence that impairment played a role in the circumstances of the accident.
